This way you can get the worksheets. It looks like what is above in the picture but it has changed over the years.

This flower gives you a better grasp of how you view yourself by doing different exercises. You can mentally answers what is in the flower without reading the exercises but it will not fully express the right answers because you are not mentally going through the tasks. That's why this is so important and is more effective in understanding yourself.
